Below is a list of fixes for COD5, atleast the widespread problems. If your problem isn’t on the list below I suggjest going to CallofDuty and signing up at their forums and see if you can’t get some help there. The biggest error seems to be with d3dx9_37.dll. So follow the directions and you should be good to go.
Please note that these fixes MAY or MAY NOT work for you. But this is the common way to fix your problem. I’m sorry if it doesn’t help you.

Unfortunately for me NONE of these work for me at all, but are said to work for others.

1.*Server Authentication Failure*
a. Refresh server list
b. Try reloading your online profile “cod5 player”
c. Restart game. “This one almost always works for me”
2.*d3dx9_37.dll*
This can happen trying to launch cod:WW beta, or while loading map/s.
a. Update your DirectX 9.0 –Doesn’t work for me
b. Download the d3dx9_37.dll file and extract it to “C:\Program Files\Activision\Call of Duty - World at War Beta” Location may vary –Doesn’t work for me “This also seems like a random place for a DX file so if someone finds the correct spot to put this please tell me”
c. Extract d3dx9_37.dll into “C:\WINDOWS\system32″ –Doesnt work for me
d. Extract DirectX Redistributable 9.0c (August 2008) to “C:\WINDOWS\system32″
3.*Black Screen*
This can happen trying to launch cod:WW beta, or while loading map/s.
a. Restart comp.
b. Make sure *ALL* drivers are updated. “Includes: Soundcard “audio drivers”, Videocard”
c. Reinstall “A Last resort”
d. There has been no “Official or Possible fix as of yet”
4.*Unhandled Exception Error*
VISTA:
a.
- Go to Control Panel
- The click on “Hardware and Sound”
- Click on “Sound”
- Doble click on “speakers”
- go to the Advanced tab
- Set the default format to “24 Bit, 48000 Hz (Studio Quality)”
- Thats it!
b. Control Panel > Hardware and Sound > Sound > Speakers > Properties > Advanced Tab
change the “Default Format” to 24 bit, 48000 Hz (Studio Quality)
XP/SP3
PCI telephone modem card was causing my issue “Eskimonio”
a. My Computer > Properties > Hardware > Device Manager > Sound, vid/game Controllers > UNIMODEM Half Duplex Audio Device > DISABLE
5.*Error Creating Online profile”
This happened to me when I updated my drivers.
- a. Restart
- b. Run as administrator.
- c. Restart comp.
- d. Reinstall
6.*Error Connecting to Online Servers*
This is after it verifies your user “online player”
a. Restart game “usually doesn’t work but try”
b. Restart comp. “said to work”
c. Uninstall/Reinstall “Make sure u uninstall EVERYTHING to do with cod5 *includes all other player profiles*”
7.*Sound Fix* “Krigo”
- Go to Control Panel
- The click on “Hardware and Sound”
- Click on “Sound”
- Doble click on “speakers”
- go to the Advanced tab
- Set the default format to “24 Bit, 48000 Hz (Studio Quality)”
- Thats it!
-If you have Creative SB X-Fi then you need to install the Beta Driver dated 17th Oct
8.*Castle Error* “Help me”
This error happens when you try to load the map “Castle”
-Either it loads and there are no textures and all models are flashing “Includes: Houses/dogs/players/guns/ect.” Also there are triangles everywhere “flying”
-Completely locks down computer forcing me the manually restart “press power button”

It’s late, so I haven’t tested all maps, but it looks like it’s working for me.
Here’s what I did:
1) Dell factory reset
Install CoD5
2) Installed initial Nvidia G-carddrivers (restarted)
3) Updated service packs & other ms updates
4) (restarted)
5) Installed DirectX (latest)
6) Rebooted
7) Nvidia drivers update + restart
9) Restart
10) Patch
11) Restart
I haven’t checked SP, but MP loading maps seems fine :D”

but it Is So Slow i am using the Lowest Options what more do i have to do?

when i have finished blowing up the two tanks level it starts to load next level and then go,s back to desk top and closes its self down any ideas getting sick of only being able to play the three levels .platform pc
britishbulldog - Im having the same problem as you mate. It either crashes while loading that map or it crashes when I click to start the game. Do you also have sound issues when it is loading? Because the speech on mine turns on and off. Any help would be great
For the Handling exception error, unistall and preferably remove your onboard phone modem, then it will load, judt disabling will not work you must uninstall …hope this helps

it is a Directx issue. the best fix is a reinstall of dx 9.0 by using the uninstaller program, then reinstalling it, if you need to do this google is your friend and will set you up with everything you need
I can’t connect to any servers
sometimes I see one or two, but when I am able to connect (rarely), it’s near impossible to play on them
My connection is perfect, and runs COD4 with no lag
What gives?
Anyone having the issue where Cod runs at hyper speed in small spaces, or when looking down. then everything else is off time? Think it might be Direct X related then again I don’t know.

Also, it was only a partial fix, 2 MP maps still don’t load, one being Asylum. This is an improvement, but still…
On SP, I’m able to play a further 5 levels, which takes me up to Berlin before it crashes (the same issue as britishbulldog).
Advice: Haunt the Treyarch and CoD forums like a son-of-a-b1tch-ghost. There WILL be a solution, either a community one, or a Treyarch one. We just have to be patient. Oh - and moan a lot ofcourse.
My start up black screen was fixed by uninstalling the 56k modem from the device manager. Stupid simple but it worked(try it)
i have no audio devices cant change to studio quality

ARGH THIS IS ANNOYING!!! hey - the person who couldnt load water droplet and dynamic spray - i have seen this problem on a friends computer, fix by reinstalling, maybe try to download a new image as the one you have may be corrupted?
I have XP and COD5 failed to load up at all. I tried most fixes but the one that got it in the end was to disable the 56k modem.

Ok, i have a problem. When i start the game, it goes to a black screen and then closes and opens the COD console.
It says:
ERROR: image ‘images/loadscreen_sp_ber3b.iwi’ is missing
Couldn’t load image ‘loadscreen_sp_ber3b’
Can sum1 upload this file from their game istalation by going to:
C/Program Files/Activision/Call of Duty - World at War/main/
In there there will be a whole load of IWD files labeled iw_00 - iw_16
Open “iw_04″ with WinRAR and find the iwi file called ‘loadscreen_sp_ber3b’. Then extract it and upload it somewhere like rapidshare or megaupload. Then post the link here.
